The
Geriatric Day Centre serves the frail elderly by providing programs
and services for non-residents. Working with other community services,
we strive to enable the elderly still living at home to continue to
do so for as long as possible.
Our goals are to:
- Maintain or improve the participants' level of functioning;
- Enhance their quality of life;
- Decrease isolation;
- Provide respite for their caregivers.
The
Day Centre uses an interdisciplinary approach, calling on the expertise
of several staff, including:
- Nurse/coordinator
- Recreation technician
- Special care counsellor
- Nursing assistant
- Nurse's / dietary aide
- Physiotherapy technician
- Occupational therapist
In order to be admitted
to the Geriatric Day Centre, participants must be:
- At least 60 years
of age, or have a geriatric profile;
- Living at home or
in a community setting;
- In need of a structured
program for cognitive, physical or social
reasons;
- Willing to attend
the Day Centre on a voluntary basis.
Referrals are usually made
through CLSC case managers or other health care professionals. |
